It is a good idea to allow yourself some extra time to check in and complete paperwork by arriving early for your consultation appointment. Please remember that a parent or legal guardian must accompany a patient under the age of 18.

Please bring the following with you:

  • Referral slip from a dentist or referring provider
  • Health insurance
  • Dental insurance
  • Current medications list (heart medications, aspirin, anticoagulant therapy, etc.)
  • Driver’s license or another valid form of identification
  • Form of payment (more information can be found below in the Financial & Insurance Policy section)

For your health and safety, please inform us of any current and/or prior medical conditions such as diabetes, high blood pressure, artificial valves and joints, heart conditions, etc.
